Classification of Emergencies

Introduction and Principles of Emergency Classification

Emergency classification is the systematic process of sorting patients based on the urgency of their medical needs and the resources required to save their lives. In busy Nigerian healthcare settings, where patient loads often overwhelm available staff and equipment, proper sorting prevents avoidable mortality. Nurses act as the first line of defense, swiftly recognizing clinical deterioration. Sorting ensures that a patient with a tension pneumothorax receives immediate intervention ahead of someone with a simple laceration.

Triage Systems and Priority Levels

Triage structures the chaos of the emergency department. Systems like the South African Triage Scale (SATS) use physiological vital signs and presenting complaints to assign a triage color and maximum waiting time.

Medical and Trauma Emergencies

Medical emergencies arise from internal system failures, whereas trauma emergencies stem from external physical forces. Nurses must quickly differentiate between these two broad categories to initiate appropriate care protocols.

Obstetric, Gynecological, and Pediatric Emergencies

Special population emergencies require distinct knowledge and rapid escalation due to the high risk of rapid clinical decline.

Psychiatric and Behavioral Emergencies

Psychiatric emergencies involve acute disruptions in thought, mood, or behavior that threaten the safety of the patient or others. Common presentations include acute psychosis, severe depression with suicidal intent, and violent agitation due to substance intoxication. The nurse's priority is ensuring a safe environment, de-escalating tension, and preventing self-harm while preparing the patient for medical evaluation.

Mass Casualty Incidents and Disaster Classification

A Mass Casualty Incident (MCI) occurs when the volume of patients overwhelms local healthcare resources and personnel. Unlike standard emergency care where staff treat the sickest patients first, disaster nursing uses specialized sorting frameworks like START (Simple Triage and Rapid Treatment). START categorizes victims using respiration, perfusion, and mental status (RPM) to quickly tag victims for immediate, delayed, minor, or expectant management.
