Vesicovaginal Fistula

Introduction and Definition of Vesicovaginal Fistula

A vesicovaginal fistula (VVF) is an abnormal opening or tract between the urinary bladder and the vagina. This abnormal connection results in continuous, involuntary leakage of urine into the vaginal vault. For affected women in Nigeria, this condition strips away dignity due to the constant wetness and associated ammoniacal odor. It is one of the most distressing complications of childbirth, leaving deep physical and social scars if left untreated.

Aetiology and Risk Factors in the Nigerian Context

Prolonged obstructed labor remains the leading cause of vesicovaginal fistula in Nigeria, accounting for the vast majority of cases. During obstructed labor, the fetal head compresses the soft tissues of the bladder and vagina against the maternal pubic bone. This sustained pressure cuts off blood supply, leading to ischemic necrosis and subsequent tissue sloughing.

Clinical Presentation and Diagnostic Evaluation

Diagnosing a vesicovaginal fistula relies heavily on a thorough clinical history and physical examination. Patients typically present with a history of prolonged labor followed by continuous urinary leakage starting a few days postpartum.

During evaluation, a sterile speculum examination is performed in the lithotomy position to visualize the vaginal walls and locate the fistula opening. If the opening is small or difficult to see, the three-swab test is utilized: methylene blue dye is instilled into the bladder via a catheter, and three vaginal swabs are placed at different levels. Staining of the upper or middle swab confirms a vesicovaginal fistula, while staining of only the lowest swab points toward ureteric injury or stress incontinence.

Pre-operative Nursing Assessment and Management

Comprehensive pre-operative care prepares the patient physically and emotionally for corrective surgery. Nurses must assess the patient's nutritional status, as many affected women are malnourished and anemic due to chronic illness and social neglect.

Skin integrity requires close monitoring because continuous urine leakage causes severe vulvar excoriation, ammoniacal dermatitis, and ulceration. Nurses apply barrier creams and encourage frequent washing. Providing empathetic counseling is vital, as these women often experience severe depression, social isolation, and stigmatization within their communities.

Surgical Intervention and Post-operative Care

Surgical closure is the definitive treatment for vesicovaginal fistula. The primary goal is tension-free closure of the bladder and vaginal walls, separating the two structures. Post-operative nursing care focuses on maintaining continuous bladder drainage to keep the repair site free of tension while it heals.

Psychosocial Impact, Rehabilitation, and Prevention

Physical closure of the fistula is only half the battle; complete recovery requires addressing the deep psychological and social consequences. Many patients face abandonment by spouses, rejection by family members, and severe economic hardship. Nurses coordinate community reintegration programs, psychological counseling, and skills acquisition training to help survivors rebuild their lives.

Prevention remains the ultimate public health solution. Expanding access to skilled birth attendants, promoting girl-child education, delaying marriage, and ensuring timely referrals for obstructed labor through functional primary health care systems can eliminate vesicovaginal fistula across Nigeria.
